Name Change Thread / v.4

Welcome to Version 4 of our CanvasForums Name Change Thread!

We are now allowing CanvasForums users to change their name to a name of their choosing, after
successfully registering and being verified by forum staff. To request a name change, please fill out
and post a form, from below) to get the process started.

Our registration and profile forms have changed. We have requirements for making a name change.
To keep your name changed, and your warning bars full, follow these simple rules:

You must have your IMVU User ID and your IMVU username present on your profile.
See spoiler for a visual representation. You must keep your IMVU name updated, to keep your custom forum name.

You must have a filled out forum profile.
Uploaded DP, signature, and something in the "About Me" box.

The staff of CanvasForums reserves the right to decline any name change request.
